A failing heart is not primarily a weak muscle but an energy-starved one. Myocardial tissue consumes more adenosine triphosphate per gram than any other organ, and in heart failure the machinery that regenerates it works less efficiently, which is the observation that has kept mitochondrial cofactors in the research literature for four decades. The clinical syndrome affects 6.7 million US adults and about 10 percent of those over 65, and presents as breathlessness on exertion and later at rest, orthopnoea, paroxysmal nocturnal dyspnoea, peripheral oedema, fatigue and exercise intolerance.
Seven ingredients are graded against this condition, on 42 studies and 8,650 participants. Two reach Grade B, two Grade C and three Grade D.
Coenzyme Q10 has the longest history of the group and holds Grade B on 15 studies and 1,500 participants. It is an obligatory electron carrier in the respiratory chain and a lipid-soluble antioxidant, and myocardial concentrations fall as functional class worsens. Reviews specific to heart failure have assessed both the mechanistic rationale and the trial evidence [1], including sceptical appraisals of whether the endpoint data justify the mechanistic enthusiasm [2]. Formulation is a live issue, since the reduced form, ubiquinol, and the oxidised form, ubiquinone, differ in absorption, and the comparison has been examined directly [3]. A second consideration recurs in this population: statin therapy lowers circulating coenzyme Q10 as a consequence of the same enzymatic inhibition that lowers cholesterol.
Hawthorn holds the other Grade B rating, on 14 studies and 3,800 participants, the largest study-and-participant combination for any botanical on the page. Its extracts contain oligomeric procyanidins and flavonoids, and the trials have generally used standardised preparations with symptom-based and exercise-capacity endpoints.
Vitamin D is graded C with a mixed direction on 6 studies and 3,000 participants. Low vitamin D status is common in heart failure, but the causal direction is contested, since reduced outdoor activity is a consequence of the syndrome as well as a candidate contributor to it. Reviews of vitamin supplementation in this population have addressed that ambiguity explicitly [4]. Taurine completes the Grade C tier on 4 studies and 200 participants, supported by its role in myocardial calcium handling.
The three Grade D entries illustrate the range within a single grade. L-arginine has 3 studies, 150 participants and a mixed direction. NMN and PQQ are recorded with zero studies and zero participants against this endpoint: their inclusion rests on mechanistic reasoning about mitochondrial biogenesis and NAD metabolism, not on trial data, and the grade states as much.
Heart failure is also the cardiovascular condition where interactions carry the greatest consequence, because standard therapy involves diuretics, renin-angiotensin blockade and rhythm-active drugs simultaneously. Sodium and fluid balance, potassium status and renal function are all under active pharmacological management, which narrows the margin available to anything taken alongside it.
