For most of the twentieth century the inner lining of a blood vessel was regarded as inert plumbing. It is now understood as a continuous endocrine surface, one cell thick, that senses shear stress and responds by releasing nitric oxide, prostacyclin and endothelin to regulate vessel tone, platelet adhesion and leukocyte traffic. Endothelial dysfunction describes the loss of that regulatory capacity, most often measured as impaired flow-mediated dilation, and it is present in virtually all patients carrying cardiovascular risk factors. It precedes visible structural disease, which is why it is regarded as the earliest detectable stage of the atherosclerotic process rather than a separate condition.

It is subclinical by definition. The listed manifestations, reduced exercise tolerance, erectile dysfunction and impaired wound healing, are downstream consequences of inadequate flow reserve rather than symptoms of the endothelium itself. Erectile dysfunction is the most informative of the three, because the penile arteries are small enough that a given degree of endothelial impairment restricts them before it restricts the coronary vessels, making the symptom an early sentinel in men.

Four ingredients are graded here, on 49 studies and 26,050 participants. Cocoa flavanols hold the only Grade A rating in the set, and by some distance: 30 studies and 25,000 participants, which is the great majority of the participant total on this page. The mechanism has been characterised more precisely than for most food-derived compounds. Flavanol monomers and their colonic metabolites increase nitric oxide availability in endothelial cells, and flow-mediated dilation responds within hours of ingestion, which gives trials a fast, quantitative and repeatable endpoint rather than a symptom score. That measurability is part of why this evidence base is unusually large.

L-arginine holds Grade C on 11 studies and 600 participants. As the substrate for nitric oxide synthase it has the most direct rationale of any ingredient on the page, yet the clinical results have been inconsistent, partly because the enzyme is not normally substrate-limited in healthy tissue and partly because arginine is extensively metabolised by arginase before it reaches the enzyme. Preclinical work on nitric-oxide-mediated vasorelaxation, including studies in isolated rat aortic rings using other plant-derived compounds, has continued to probe the same pathway [1].

Grape seed extract, at Grade C on 6 studies and 350 participants, contains oligomeric proanthocyanidins related to the cocoa flavanols but has been examined at a fraction of the scale. Astaxanthin completes the set at Grade D on 2 studies and 100 participants; the case for it rests on carotenoid antioxidant activity within the cell membrane, reviewed in the cardiovascular context some years before those trials were run [2].

All four are recorded as positive in direction. Smoking, diabetes, hypertension, dyslipidaemia, ageing, obesity, inactivity and oxidative stress remain the drivers, and each impairs the same nitric oxide pathway that the ingredients above are intended to support.
