Magnesium holds Grade B on 8 studies and 500 participants, with a positive direction. Its plausibility rests on established electrophysiology: magnesium is a cofactor for the sodium-potassium ATPase and modulates several potassium currents, and depletion shortens the refractory period. Much of the clinical work has concerned the specific setting of atrial fibrillation following cardiac surgery, where the exposure is short, monitored and measurable [2].

Potassium is graded C on 5 studies and 300 participants. The interest is mechanistic rather than nutritional: serum potassium sets the resting membrane potential, and controlled elevation within the normal range has been examined for its effect on repolarisation in long QT syndrome [3]. This is also the ingredient on the page where the case for caution is strongest. Potassium has a narrow therapeutic window; hyperkalaemia is itself arrhythmogenic, and the risk concentrates in people with impaired renal clearance, whose outcomes have been documented in population cohorts of chronic kidney disease [4]. Coenzyme Q10 completes the set at Grade C on 4 studies and 300 participants.

The listed risk factors point away from supplementation for most cases. Hypertension, coronary artery disease and heart failure are structural contributors. Thyroid disorders alter the electrical substrate systemically. Caffeine and alcohol excess, sleep apnea and electrolyte disturbance are provocations that are modifiable without a supplement.
